Hospice patients in financial need and their immediate family may receive help from CCHFoundation throughout 24 states which include California, Delaware, Florida, Georgia, Illinois, Kansas, Louisiana, Maine, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, Nebraska, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, South Carolina, South Dakota, Texas, Virginia and Wisconsin. CCHF also assists hospice patients and families living in
in-patient units, nursing homes, prison, and hospitals.
More than 65,000 terminally ill patients and their loved ones of any age, gender, sexual orientation, religious and ethnic background have received assistance since CCHF's founding in April of 2004. To be eligible for assistance an individual must always be under hospice care and have exhausted all available community resources, fall within federal poverty guidelines or have extraordinary financial circumstances.
NEW PROGRAMS ADDED!
Honoring All Who Served CCHF has added a special Veteran's account for Vets who are in serious financial or emotional need. Typically one-time only purchase of small things like sheets, haircut, toiletries, pajamas, a birthday cake, a special lunch or dinner, etc. may be purchased.
These dollars are not to be used for the larger items listed on the CCHF request form – utilities, rent, mortgage, etc… that should be made as a formal request by the patient's social worker.
Pat Heiland is a longstanding Board Member of CCHF and a 2016 retiree of Compassionate Care Hospice. Pat will be remembered for the loving care given to patients and their pets living in the Pennsylvania area. She consistently provided food, veterinarian care and placement assistance for hospice patients who were in danger of loosing their loving pet companions. In her honor, CCHF has established the PAT'S PETS fund to be used to continue her kindness on a national level!
