NOTE TO SELLER. Give us the name of the patient or any special instructions in this area. This will allow us to notify the family of your kindness. We send the family notification in several weeks in an acknowledgement letter - if you would like us to send a SYMPATHY CARD as well, please indicate that in the Note to Seller section.
Holding a special event is another wonderful and creative way to make the community aware of CCHF while raising funds too!
CCHF has been the recipient of several creative donations. In one instance, wedding guests were asked to donate to CCHF in lieu of more material wedding gifts. Some have made gifts in honor of a birthday or other special occasions. Still others have donated to the Pet Therapy Program in memory of a beloved pet. You may have yet another creative way to help others in a special time of need.

DONOR PRIVACY POLICY
CCHF does not copy, reproduce, publish, post, distribute, share or sell, the name, personal contact information or description of donation of donor to any outside individual or business. Exceptions shall be those required by law, requested in grant applications or in connection with bank credit or debit card transactions that may be made without prior knowledge or consent of the donor. CCHF does acknowledge donors by name and address in acknowledgment letters to bereaved families unless specifically requested by a donor. However, no dollar amount is listed with regard to the donation in that letter. CCHF does acknowledge donors by name and/or description of donation but not dollar value, after the donors have given their written permission, in certain printed material such as chapter newsletters, brochures, press releases, event invitations or programs, stationery, solicitations letters, the organization’s website or other means or expressing appreciation for the donors’ assistance. A donor wishing to view samples of these publications may do so by contacting CCHF. CCHF provides written communication regarding this matter on all thank you letters. Contributors are asked to contact CCHF if they do NOT wish to have their donation publicly acknowledged. All donations shall be treated individually so that multiple donations from a single donor shall receive the same consideration as if each were the first donation received. If a donor chooses to opt out of any acknowledgement listing in printed material, the donor’s name and address will be made available only to members and/or staff of CCHF who are responsible for managing the business of the corporation. A printed copy of CCHF Donor Privacy Policy will be mailed to a donor upon request and is also available on the corporation’s website. All records of donations to CCHF shall be maintained by the corporation according to the applicable standards of record retention in effect at the time of the donation. All records will be maintained in a secure location at CCHF. A donor’s photograph will not be used at any time or in any CCHF publication without the donor’s written consent. In the case of a minor, written permission will be obtained from theparent/guardian. The personal information of a CCHF contributor will not be shared without the written consent of the individual.
